國立雲林科技大學圖書館借書證辦理要點 National Yunlin University of Science and Technology Guidelines for the Application of Library Borrowing Cards

一、 本校圖書館(以下簡稱本館)為服務退休教職員工、畢業校友、現職教職員工眷屬及支援教育推廣班教學、兼任教師及駐校藝術家使用,特訂定本要點。

二、 借書證之辦理:

(一)退休教職員工
凡退休教職員工,均可持身分證正反兩面影本1份、2吋近照1張及退休證明相關證件、申請單1份等,於開館時間內親自到本館辦理借書證;或於本館網站進行申 辦,並請依網站說明完成繳費,方可辦證。不合乎規定者,本館予以退件。辦證須繳交保證金新臺幣2000元,該保證金得由本校現仍在職之正式編制內教職員工具保書替代,惟憑具保書申辦者需親臨櫃台申辦。借書證有效期間為1年,保證金於退回借書證後申請發回。
(二)畢業校友
凡畢業校友,均可持身分證正反兩面影本1份、2吋近照1張、申請單1份等,於開館時間內親自到本館辦理借書證;或於本館網站進行申辦,並請依網站說明完成繳費,方可辦證。不合乎規定者,本館予以退件。辦證須繳交借書證工本費新台幣300元(含續辦)及保證金新台幣2000元,該保證金得由本校現仍在職之正式編制內教職員工具保書替代,惟憑具保書申辦者需親臨櫃台申辦。借書證有效期間為1年,保證金於退回借書證後申請發回。
(三)教學單位辦理之推廣教育班師生
由承辦單位統一簽證辦理借書證,需繳交借書證每張工本費新台幣300元,借書證有效期限以修業期間為主。個人辦理需繳交保證金2000元及借書證每張工本費新台幣300元,讀者可於使用到期後,還清所借之圖書及罰款,始得無息退還保證金。
(四)教職員眷屬
年滿6歲可憑教職員工證正面影本1份、本人戶口名簿影本1份、2吋近照1張、申請單1份等,於開館時間內到本館辦理借書證;或於本館網站進行申辦,並請依網站說明完成繳費,方可辦證。不合乎規定者,本館予以退件。辦證須繳交借書證工本費新台幣300元(含續辦),借書證有效期間為1年。
(五)兼任教師、駐校藝術家
凡兼任教師、駐校藝術家,均可持身分證正反兩面影本1份、2吋近照1張、本校正式聘書、申請單1份等,於開館時間內親自到本館辦理借書證;或於本館網站進行申辦,並請依網站說明完成繳費,方可辦證。不合乎規定者,本館予以退件。第1次辦證(或證件遺失汙損須重新印製者)須繳交借書證工本費新台幣300元及保證金新台幣2000元,該保證金得由本校現仍在職之正式編制內教職員工具保書替代,惟憑具保書申辦者需親臨櫃台申辦。借書證有效期間以聘書期間為主,保證金於退回借書證後申請發回。

三、 借閱人如具雙重身分,自行擇一身分辦理借書,不得重複申領借書證。

四、 本要點經圖書委員會議通過,陳請校長核定後實施,修正時亦同。

  1. These regulations are established by the University Library (hereinafter referred to as “the Library”) to provide library services to retired faculty and staff, alumni, family members of current staff, continuing education program instructors and students, part-time faculty, and artists-in-residence.

  2. Library Card Application:

    (1) Retired Faculty and Staff
    Retired faculty and staff may apply for a library card in person during opening hours by submitting a photocopy of both sides of their national ID card, one recent 2-inch photo, proof of retirement, and a completed application form. Applications can also be submitted online via the Library’s website, and payment must be completed as instructed. Incomplete applications will be rejected. A security deposit of NT$2,000 is required. This deposit may be waived if a current full-time faculty or staff member of the university provides a letter of guarantee. In such cases, the application must be submitted in person. The library card is valid for one year. The deposit will be refunded upon return of the card.

    (2) Alumni
    Alumni may apply for a library card by presenting a photocopy of both sides of their national ID card, one recent 2-inch photo, and a completed application form in person during opening hours. Applications can also be submitted online via the Library’s website, and payment must be completed as instructed. Incomplete applications will be rejected. Applicants must pay a card issuance fee of NT$300 (including renewals) and a security deposit of NT$2,000. The deposit may be waived with a letter of guarantee from a current full-time faculty or staff member. In such cases, the application must be submitted in person. The library card is valid for one year. The deposit will be refunded upon return of the card.

    (3) Continuing Education Program Instructors and Students
    The responsible academic unit shall handle the application process collectively, and a fee of NT$300 per card will be charged. The validity of the card corresponds to the duration of the program. Individual applicants must pay both a NT$2,000 deposit and a NT$300 issuance fee per card. The deposit will be refunded without interest after all borrowed items are returned and any fines are paid.

    (4) Family Members of Current Staff
    Family members aged six or above may apply by presenting a copy of the front side of the staff ID card, a household registration copy, one recent 2-inch photo, and a completed application form. Applications may also be submitted online with required payment. Incomplete applications will be rejected. A card issuance fee of NT$300 (including renewals) is required. The card is valid for one year.

    (5) Part-time Faculty and Artists-in-Residence
    Applicants must submit a photocopy of both sides of their national ID card, one recent 2-inch photo, a valid employment contract issued by the university, and a completed application form, either in person during opening hours or online via the Library’s website with completed payment. Incomplete applications will be rejected. First-time applicants (or those requesting replacement cards due to loss or damage) must pay a NT$300 issuance fee and a NT$2,000 deposit. The deposit may be waived with a letter of guarantee from a current full-time faculty or staff member. In such cases, the application must be submitted in person. The validity of the card corresponds to the term of the contract. The deposit will be refunded upon return of the card.

  3. Individuals with dual eligibility (e.g., retired faculty and alumni) must choose one status for application and may not hold more than one card.

  4. These regulations shall take effect upon approval by the Library Committee and ratification by the President. Amendments shall follow the same procedure.
