一、 雲林科技大學圖書館(以下簡稱本館)為服務退休教職員工、畢業校友、現職教職員工眷屬及支援教育推廣班教學、兼任教師及駐校藝術家使用,特訂定本要點。
二、 借書證之辦理:
(一)退休教職員工
凡退休教職員工,均可持身分證正反兩面影本1份、2吋近照1張及退休證明相關證件、申請單1份等,於開館時間內親自到本館辦理借書證;或於本館網站進行申 辦,並請依網站說明完成繳費,方可辦證。不合乎規定者,本館予以退件。辦證須繳交保證金新臺幣2000元,該保證金得由本校現仍在職之正式編制內教職員工 具保書替代,惟憑具保書申辦者需親臨櫃台申辦。借書證有效期間為1年,保證金於退回借書證後申請發回。
(二)畢業校友
凡畢業校友,均可持身分證正反兩面影本1份、2吋 近照1張、申請單1份等,於開館時間內親自到本館辦理借書證;或於本館網站進行申辦,並請依網站說明完成繳費,方可辦證。不合乎規定者,本館予以退件。辦 證須繳交借書證工本費新台幣300元(含續辦)及保證金新台幣2000元,該保證金得由本校現仍在職之正式編制內教職員工具保書替代,惟憑具保書申辦者需 親臨櫃台申辦。借書證有效期間為1年,保證金於退回借書證後申請發回。
(三)教學單位辦理之推廣教育班師生
由承辦單位統一簽證辦理借書證,需繳交借書證每張工本費新台幣300元,借書證有效期限以修業期間為主。個人辦理需繳交保證金2000元及借書證每張工本費新台幣300元,讀者可於使用到期後,還清所借之圖書及罰款,始得無息退還保證金。
(四)教職員眷屬
年滿6歲可憑教職員工證正面影本1份、本人戶口名簿影本1份、2吋近照1張、申請單1份等,於開館時間內到本館辦理借書證;或於本館網站進行申辦,並請依網站說明完成繳費,方可辦證。不合乎規定者,本館 予以退件。辦證須繳交借書證工本費新台幣300元(含續辦),借書證有效期間為1年。
(五)兼任教師、駐校藝術家
凡兼任教師、駐校藝術家,均可持身分證正反兩面影本1份、2吋近照1張、本校正式 聘書、申請單1份等,於開館時間內親自到本館辦理借書證;或於本館網站進行申辦,並請依網站說明完成繳費,方可辦證。不合乎規定者,本館予以退件。第1次 辦證(或證件遺失汙損須重新印製者)須繳交借書證工本費新台幣300元及保證金新台幣2000元,該保證金得由本校現仍在職之正式編制內教職員工具保書替 代,惟憑具保書申辦者需親臨櫃台申辦。借書證有效期間以聘書期間為主,保證金於退回借書證後申請發回。
三、 借閱人如具雙重身分,自行擇一身分辦理借書,不得重複申領借書證。
四、 本要點提經圖書委員會議通過,陳請校長核定後實施,修正時亦同

1. These guidelines are established by the National Yunlin University of Science and Technology Library (hereinafter referred to as "the Library") to provide borrowing privileges to retired faculty and staff, alumni, dependents of current faculty and staff, students and teachers in continuing education programs, part-time lecturers, and artists-in-residence.
2. Procedures for applying for borrowing cards:
(1) Retired Faculty and Staff
Eligible applicants must provide:

A copy of both sides of their national ID

One 2-inch recent photo

Proof of retirement

Completed application form
Applicants may apply in person during Library opening hours or submit their application through the Library website and complete the required payment as instructed. Incomplete applications will be rejected. A deposit of NT$2,000 is required, which may be waived if a current full-time faculty or staff member of the University provides a signed guarantor form. Applications with guarantor forms must be submitted in person. The borrowing card is valid for one year. The deposit will be refunded upon return of the borrowing card.
(2) Alumni
Applicants must provide:

A copy of both sides of their national ID

One 2-inch recent photo

Completed application form
Applications may be submitted in person or online via the Library website, with payment completed as instructed. Incomplete applications will be rejected. A card processing fee of NT$300 (also applicable to renewals) and a deposit of NT$2,000 are required. The deposit may be waived with a guarantor form provided by a current full-time faculty or staff member; such applications must be submitted in person. The borrowing card is valid for one year, and the deposit will be refunded upon return of the card.
(3) Students and Teachers in Continuing Education Programs
Applications must be submitted collectively by the responsible academic unit. A card processing fee of NT$300 per card is required. The card is valid for the duration of the program. Individual applicants must pay both the NT$300 processing fee and a NT$2,000 deposit. The deposit will be refunded without interest after all borrowed items and fines are cleared.
(4) Dependents of Faculty and Staff
Applicants aged 6 or above may apply with:

A copy of the front side of the current faculty/staff ID

A copy of the applicant's household registration

One 2-inch recent photo

Completed application form
Applications may be submitted in person or online with completed payment per website instructions. Incomplete applications will be rejected. A card processing fee of NT$300 (also applicable to renewals) is required. The card is valid for one year.
(5) Part-Time Lecturers and Artists-in-Residence
Applicants must provide:

A copy of both sides of their national ID

One 2-inch recent photo

A valid appointment letter issued by the University

Completed application form
Applications may be submitted in person or online, with payment completed as instructed. Incomplete applications will be rejected. First-time applicants (or those replacing lost/damaged cards) must pay a processing fee of NT$300 and a deposit of NT$2,000. The deposit may be waived with a guarantor form provided by a current full-time faculty or staff member; these applications must be submitted in person. The card is valid for the duration of the appointment, and the deposit will be refunded upon return of the card.
3. Individuals with multiple eligible statuses may apply under only one selected category. Multiple applications are not permitted.
4. These guidelines shall take effect upon approval by the Library Committee and ratification by the University President. Any amendments shall follow the same procedure.
